Out of Network Prescription Coverage

If you have eye insurance like VSP, MES, EyeMed, you can submit your receipt from Blenders for reimbursement as long as you haven’t used your benefits for the year yet. To do so, first find out which eye insurance you have. Secondly, download their “Out-of-network reimbursement” form from the links provided below. With a copy of your receipt from Blenders and a filled out form, mail that into your provider. Within a few weeks that provider will send you a check for the amount your insurance reimburses for out-of-network claims.
